PICC Line Order Form

Hi everyone,

We are looking to update our current Physician Central Line Order
Sheet as we have noticed that drug pH and/or osmolarity are overlooked
when physicians are ordering medications resulting in administration
via an inappropriate IV line.

We are proposing to include an area where the physician would need to

RE: [ASMSO] IV fluid hang times/beyond use dating

ASMSO Email List Server - Thu, 02/19/2009 - 17:42
So sorry... I tried creating a table in my original post and I see it
turned out to be a big mess...

I had 3 columns: the name of the IV product, the "beyond use date" (for
non pre-mixed products), and the hang time. Below is the list of
products:

1. Pre-mixed bag of clear fluid (e.g. 0.9% NS)

Hello everyone, I am finishing up a MUE on Erbitux at a local hospital
here and was wondering if you had any protocols or physician order forms
which ensure the appropriate prescribing and monitoring of Erbitux.
Thanks.

IV fluid hang times/beyond use dating

ASMSO Email List Server - Thu, 02/19/2009 - 17:42
* How do folks determine expiration dates/beyond use dating/hang
times, whatever you want to call it, for IV drips? Does your policy
specify a "beyond use date" for the product (per USP, below) and a
maximum "hang time"? How did you determine the hang time for
preparations other than TPN and lipids (these 2 are addressed in CDC

RE: [ASMSO] hang time for clear fluids

ASMSO Email List Server - Wed, 02/18/2009 - 21:42
I found this from the CDC:

Guidelines for the Prevention of Intravascular Catheter-Related
Infections; August 9, 2002 / 51(RR10);1-26

Parenteral fluids

1. Complete the infusion of lipid-containing solutions (e.g., 3-in-1
solutions) within 24 hours of hanging the solution (156--158,226,229).

I'd have to take some time to dig up the info, but the CDC about 3 years ago produced some material on this issue.
The piece indicated that premanufactured bags and IV tubing are safe up to 96 hours.

hang time for clear fluids

ASMSO Email List Server - Wed, 02/18/2009 - 19:42
I noticed that our pre-op area will spike and prime their bags of NS and
LR in advance and give them an expiration of 96 hours. This doesn't seem
OK, but I cannot find a reference to support my idea.
USP 797 talks about low-med-high risk IV meds that are "compounded".
Would a spiked bag of NS be considered "compounded"?

Hello everyone,
I am trying to find information about the treatment of vancomycin and

have policies in place for these non-vesicant drugs.

Thank you!
