The Medication Use Safety Pharmacy Residency Program is a postgraduate training program that focuses on the development of clinical, analytical, and leadership skills for future leaders in healthcare. The Residency offers advanced training in patient safety best practices and emerging technologies at UMass Memorial Medical Center with an emphasis on clinical applications and impact on patient care. This specialized residency is a 1-year residency designed to foster knowledge and appreciation for the opportunities in a hospital pharmacy practice setting for patient safety and automation. The resident will gain experience with the implementation and management of National Patient Safety Goals, ADE surveillance, Computerized Prescriber Order Entry (CPOE), smart pump management; controlled access cabinets (Pyxis), RFID, point of care barcoding, clinical documentation, and PADE rules based alerts engine. Finally, the resident will have the opportunity to work with nationally recognized leaders in patient safety and further develop personal leadership skills.

The Medication-Use Safety Residency is sponsored by The Johns Hopkins Department of Pharmacy in collaboration with the American Society of Health-System Pharmacists. The goal of this residency is to prepare the candidate to assume a leadership role in devising and implementing process improvements that improved patient care safety. Through this unique program, residents gain experience in various methodologies used to measure medication-use safety, participate in the application of technologies that improve the medication-use process, learn and apply methods to promote a culture of safety, analyze complex systems to identify and correct system flaws, develop effective communication techniques and understand the many human-factors that impact the safety of patient care. Residents completing this program will be positioned well to function as a Medication Safety Coordinator in a heath-system setting.

Clement J. Zablocki Veterans Affairs Medical Center
The medication safety resident will work as part of an interprofessional team focusing on the improvement of safe medicine practices within the medical center. The team will consist of the pharmacy resident, a graduate nurse fellow, and a physician. The resident will receive training in performance improvement processes including health care failure modes effects analysis, root cause analysis, LEAN and Six Sigma. These processes will be used throughout the year to evaluate and develop solutions to medication safety issues identified within the institution. The resident will work with various interdisciplinary committees in the institution that focus on different aspect of the medication use process. Teaching opportunities will also be available at the Concordia School of Pharmacy.

Graduates of this residency program will have experience coordinating the development and implementation of safe medication guidelines for pharmacists, nurses and physicians, facilitating technology adoption, participating in the development of new technology for medication safety, and serving as a company-wide resource for pharmacotherapy and medication safety needs. Residents will have ample opportunity to develop leadership and communication skills through interactions with executive-level administrators and health care providers. There are also opportunities for residents to precept and teach pharmacy students from the University of Tennessee, Lipscomb University, and Belmont University. Required rotations include: Medication Safety, Patient Safety, Foundational Quality and Regulatory Compliance, Pharmacy Informatics, Educational Design.

Veterans Health Administration Pharmacy Benefits Management Service and Center for Medication Safety
The medication safety and outcomes assessment resident will be responsible for the development and completion of a minimum of 2 formal residency projects suitable for implementation and publication. The resident will be a member of multidisciplinary health plan committees, interact daily with other healthcare professionals, participate in journal clubs, and present at regional and national meetings. Education will include practical training in evidence-based medicine, statistics, pharmacoepidemiology, and healthcare economics. Out-of-state and in-state travel is required.

Purdue University – IU Health Drug Information/Medication Safety PGY2 Residency
Purdue University School of Pharmacy and Pharmaceutical Sciences and IU Health offer a one-year ASHP accredited PGY2 Residency in Drug Information/Drug Use Policy. This residency program provides advanced training in an institutional-based drug information center on various topics, including the provision of drug information, formulary and drug use policy management, adverse drug reaction and medication variance evaluation and reporting, literature analysis and integration of service throughout a multi-hospital system.

PGY2: Clinical Quality and Medication Safety Program at the University of Florida
The postgraduate training program at the University of Florida College of Pharmacy and College of Medicine emphasizes the development clinical quality programs that focus on medication safety and evidence based practices. Trainees will gain skills in consultant pharmacy and leadership both in the ambulatory and hospital settings. This specialized residency is a 1-year program with an optional 2nd fellowship year.

ISMP’s Safe Medication Management Fellowship programs are unique 12-month learning experiences beginning each July that educate a healthcare practitioner in error prevention and safe medication use methods. Fellows are based in ISMP’s Horsham, PA, office near Philadelphia, and learn and work collaboratively with leading experts to develop and implement interdisciplinary error-prevention strategies that focus on system analysis and effectively reduce the risk of medication errors.

This fellowship program is a joint effort between the Institute for Safe Medication Practices (ISMP) and the U.S. Food and Drug Administration (FDA), Center for Drug Evaluation and Research, Office of Surveillance and Epidemiology, Division of Medication Error Prevention and Analysis (DMEPA). Beginning each July, the fellow will spend 6 months at the ISMP in Horsham, PA and 6 months at the FDA with DMEPA in Silver Spring, MD.

Medication Safety Pharmacy Fellowship at Saint Vincent Hospital in Worcester, Massachusetts
This fellowship is offered in partnership with the Massachusetts College of Pharmacy and Health Sciences- School of Pharmacy-Worcester/Manchester (MCPHS-W/M). Fellowship programs at MCPHS-W/M are established to provide high-quality, post-graduate educational opportunities for motivated individuals seeking advanced exposure to academics in addition to daily immersion in the biopharmaceutical industry. Rapid growth and interest in perpetuating the MCPHS fellowship programs requires special care to ensure quality, strength and continued development. The college is committed to providing the fellow with distinctive teaching and educational experiences in a college setting and the opportunity to expand the depth and scope of their pharmacy related research skills. Activities at MCPHS-W/M balance the fellowship by providing the fellow with a research experience from an academic perspective. The purpose of the Regulatory Pharmaceutical Fellowship is to train selected candidates in one of two tracks focused on the medical and regulatory aspects of drug information dissemination or marketing. The program serves to maintain and enhance a scientific link among the Food and Drug Administration (FDA), academia, and the pharmaceutical industry; the program also provides an opportunity for the fellow to develop an understanding of the process and practices of the FDA and the pharmaceutical industry in the delivery of drug information and the regulatory oversight of prescription drug marketing practices. The fellowship will allow participants to have an opportunity to experience careers in the area of government, academia, and industry. Graduates of the program will be qualified to pursue career opportunities in FDA or other government agencies, academia or pharmaceutical industry.
The fellowship is a two-year program consisting of a 9-month rotation with FDA's Center for Drug Evaluation and Research; a 6-month rotation with Purdue University College of Pharmacy; and a 9-month rotation at Eli Lilly & Company or Johnson & Johnson Pharmaceutical Research & Development, L.L.C.

The purpose of this two-year fellowship program is to train clinical pharmacists to conduct outcomes research, including quality and safety research. AHA-NPSF Comprehensive Patient Safety Leadership Fellowship
The Fellowship consists of four in-person learning sessions, periodic teleconferences, various self and organizational assessments and individual coaching. The yearlong program culminates with the completion of an Action Learning Project (ALP) demonstrating the Fellow's ability to apply the concepts learned.
