ISMP Improving Medication Safety in Community Pharmacy: Assessing Risk and Opportunities for Change

With the release of Improving Medication Safety in Community Pharmacy: Assessing Risk and Opportunities for Change, community pharmacies now have access to a coordinated, extensive set of tools designed to reduce medication errors. The Risk document is designed to help community pharmacies take a process-driven, system-based approach to address this critical issue.

The tools were developed by the Institute for Safe Medication Practices (ISMP) with grant support from National Association of Chain Drug Stores Foundation and the AstraZeneca Medical Group.
